Community Health Centers

Community Health Centers (CHCFL) is a non-profit organization of Central Florida family health centers that provide high-quality, affordable medical, pediatric, dental and pharmacy care. Community Health Centers Forest City is located on Forest City Rd between the historic town of Eatonville and Altamonte Springs/Maitland area. This center provides comprehensive medical, dental, and specialty care for children and adults. We offer Family Medicine, Pediatrics, Adult Dental, Pediatric Dental, Behavioral Health, Optometry, Lab, and Pharmacy. In addition to preventive care, we also offer an on-site low-cost Pharmacy, exclusive to our patients.
