AxiomHealth Management

AxiomHealth Management (formerly Axiom Rehabilitation) was established in 1998, providing outpatient physical therapy in the home health setting. Two decades later, Axiom has grown to service the majority of the state of Florida. Axiom serves a large market share of the community of people with lymphedema, having developed a strong reputation for successfully managing patients with lymphedema through the Synchronized Care Program. AxiomHealth is the preferred Lymphedema partner of Managed Services Organizations (MSOs), Independent Practice Associations (IPAs), large medical groups, health plans, and other healthcare organizations across the state.
